Data Protection Policy

Thong Teck Home for Senior Citizens (“TTH”) is committed to
managing your personal data, in compliance with the Personal Data Protection
Act 2012 (“PDPA”). Thus, TTH has created this Data Protection Policy (“DPP”)
to explain how it collects, uses and discloses your personal data. This DPP
also explains how TTH protects your personal data and how you may contact TTH
about its data protection policies and procedures, as well as to exercise your
rights under the PDPA.
By providing your personal data to TTH in writing, whether through
signing our PDPA Agreement, forms, or otherwise, you are presumed and deemed to
have consented to the collection, use and disclosure of your personal data
pursuant to this DPP.
As such, please read and review this DPP carefully.
Types of Personal Data collected
Personal data under this DPP refers to personal data as defined under
the PDPA, i.e., data, whether true or not, about a person who can be identified
–
(a) from that data; or
(b) from that data and
other information to which TTH has or is likely to have access.
The types of personal data that TTH may collect include, but are not
limited to:
(a) Name;
(b) NRIC;
(c) Residential address;
(d) E-mail address;
(e) Contact details (e.g.,
contact number);
(f) Means testing
information;
(g) Medical information (e.g.,
vaccination records) and
(h) Other information
necessary to fulfil special requests (e.g., emergency contact information).
In the event that you provide any personal data to TTH relating to any
third party (e.g., providing personal data on behalf of a family member), you
confirm that you have obtained the consent of such third party for TTH to
collect, use and disclose their personal data pursuant to this DPP.
When you email or otherwise contact TTH for the purpose of receiving any
form of assistance from TTH, such as to make a request under the PDPA or to
submit a complaint, you confirm and agree to allow TTH to collect, use and/or
disclose such personal data to facilitate the provision of such assistance to
you.
Purposes for Collection, Use and Disclosure
TTH respects your privacy. TTH will not ask you for personal data unless
the collection, use and disclosure of such personal data is necessary for TTH’s
operations.
Personal data may be collected from you by our social workers, case
coordinators, and healthcare professionals (e.g, registered nurses and
therapists).
You agree that TTH may collect, use and/or disclose personal data about
you, whether obtained from you or from other sources, and/or disclose any
personal data collected from you to third parties, for the following general
purposes:
(a) Medication and medical
consultation;
(b) Transfer to other
healthcare institutions for continuity of care or in a medical emergency;
(c) Debt collection(s) and
legal purpose(s) due to debt collection;
(d) Statistical returns to
regulators and fund providers like MOH, AIC etc.;
(e) Compiling of any
relevant statistics;
(f) Application for government subsidies;
(g) Application for
healthcare, community and/or social services;
(h) Application for
assistance (financial and/or non-financial);
(i) Application for
suitable/relevant services/assistance identified/determined by staff of TTH;
(j) Verification
of information, furnished by myself and/or my family members to TTH, with
other relevant agencies and/or
individuals for services and/or assistance that I am/we are applying for at
TTH;
(k) Verification
of information furnished by other agencies and/or individuals on yourself and/or
your family members for services and/or
assistance that you are applying for with them;
(l) Where
it is necessary to disclose such information to TTH’s professional advisers,
including without limitation TTH’s
lawyers, accountants and auditors;
(m) To comply with any
legal or regulatory requirements in accordance with applicable law; and
(n) Where TTH has your
and/or your family members’ consent to do so, as the case may be.
In this regard, TTH will endeavour to ensure that your personal data is
protected by any third parties to whom your personal data is disclosed.
However, this DPP does not apply to, and we are not responsible for, the data
protection practices of these third parties.
As such, we encourage you to check these third parties’ data protection
policies to learn more about their data protection practices. Unless otherwise
precluded by the law, TTH requires third parties to protect your personal data
and abide by applicable data protection laws and regulations.
If personal data is disclosed as mentioned above, TTH will seek to limit
the scope of personal data that is furnished to the amount necessary for the
performance of the specific purpose.
Withdrawal of Consent
You may withdraw the consent that you provide for the collection, use
and disclosure of your personal data by contacting TTH’s Data Protection
Officer (“DPO”) directly at tthsc@singnet.com.sg.
Please provide us with the following for verification purposes:
(a) Full name and NRIC or
Passport number;
(b) A clear photocopy of
your NRIC or Passport;
(c) Your contact details;
(d) Name of the staff
member by whom the Personal Data was collected; and
(e) Details of the
request.
Upon receipt of your request, TTH may require a reasonable amount of
time (depending on the complexity of the request) for your request to be
processed and for TTH to notify you of the consequences regarding the same. In
general, we shall seek to process your request within thirty (30) business days
of receiving it.
Depending on the nature and scope of your request, TTH may not be in a
position to continue providing our services to you, and TTH shall, in such
circumstances, notify you before completing the processing of your request.
Should you decide to cancel your withdrawal of consent, please inform TTH in
the manner described above.
You may withdraw your consent to TTH using or disclosing your personal
data but withdrawing does not affect our rights where (1) such use or
disclosure is required by law, (2) on national/public interest grounds, (3) the
personal data in question was collected from publicly available sources or (4)
such use or disclosure is necessary in an emergency where the life or health of
a person is at risk.
Access and Correction
Likewise, you may request for access and/or correction of your personal
data in our possession or control by contacting TTH’s Data Protection Officer
(“DPO”) directly at tthsc@singnet.com.sg.
Please provide us with the following for verification purposes:
(a) Full name and NRIC or

By providing to us with complete information, we will be able to provide
you access to, update or correct your personal data as soon as reasonably
possible.

processed and for TTH to notify you of the consequences regarding the same. In
general, we shall seek to process your request within thirty (30) business days
of receiving it.

You may also make complaints in relation to your personal data in our
possession and control. For more information, please contact TTH’s Data

Complaints Process document.
Retention of Personal Data
Generally, we will store your personal data only for the time period
necessary to fulfil the purposes for which such personal data was collected, or
as required or permitted by applicable laws.
Once we determine that the retention of your personal data is no longer
necessary for those purposes, or that retention is no longer necessary for any
legal or business purpose, we will cease to retain your personal data, or
anonymise the personal data so that it can no longer be used to identify any
particular individual.
Transfer Limitation
TTH will not transfer your personal data to a country or territory
outside Singapore, except as specified in this DPP. In the unlikely scenario
that your personal data is transferred out of Singapore, TTH will take
appropriate steps to ensure that the recipients thereof are bound by legally
enforceable obligations so that your personal data continues to receive a
standard of protection that is at least comparable to that provided under the
PDPA. Such legally enforceable obligations include laws, contracts, or any
other legally binding instrument.
Variation
TTH may amend this DPP from time to time. Users are deemed to have
consented to any and all changes to the DPP from the date such changes are
published online. Thus, we encourage you to visit our website regularly to stay
updated on these changes.
Data Protection Measures
TTH maintains reasonable technical and organizational security measures
to protect personal data from loss, misuse, and unauthorized access, use,
disclosure, alteration, modification, or destruction.
For completeness, TTH takes data protection very seriously and has
strict security measures in place.
